UCT Required Forms

UCT Forms required for a student to participate in the tutorial:

• UCT Student Application 

• Parent Questionnaire and Parental Agreement

• Student Questionnaire (to be filled out by the student)

• Policies and Honor Code (parent and student agreement)

• Liability/Release Form (to be notarized by an outside resource-your bank, etc…)

• Payment Plan(payment options to teachers using post-dated checks are available by the month or semester; or by the year with a one time payment through check/venmo/paypal)
